If it falls apart without flashing that means your buddy in character 2 slot got the kill credit and not you. When you kill something in the combo, you must make sure that whatever it is flashes red and dies, not just falls apart. Make sure you are controlling character 1, hit B to team up and then kill some orcs. Start with Balin in the character 1 slot and team up with a static dwarf in character 2 (I just kept it Thorin in character 2 since he seems to be everyone's favorite).
It appears that this achievement requires you to control and form a buddy-up with each dwarf at least once and get a kill with the dwarf you are controlling in the buddy-up. I did finally get this to pop on a profile using only a single lineup of 13 dwarves instead of all 78 combinations. Okay, after working with several of my kids profiles I think I have finally figured out why this achievement seems to be so buggy.
